Why when I use pop-up alarms do I sometimes get duplicate events created?

Answer

If the alarm goes off independently in both MS Outlook and GMS Webmail with no folder synchronization having happened in between this is detected as an update clash and a duplicate event is created in your calendar.

You can configure the MS Outlook Collaboration client to change this behaviour if you wish.
